The Affordable Care Act (ACA) reporting requirements have changed, and as a result, the IRS no longer requires the company to automatically mail Form 1095-C to all employees. However, you may request a copy if you need it for your records.
If you would like to receive a copy of your Form 1095-C, you may request one by contacting:
